New Member Form
All of our new practice members will fill out the new practice member health application as completely as possible. This will allow us to understand any past injuries or traumas that may have led to current health conditions.
Consultation & Assessment
We will do a one-on-one consultation with you, where we will discuss your health concerns and goals as well as your past traumas and health history, followed by an assessment of your spine and nervous system. This allows us to provide you with the best quality of care possible.
Digital X-Rays
We will then take any and all necessary Chiropractic postural X-rays. This will allow us to look at the structure of your spine, allowing the doctor to make specific recommendations unique to your needs.
Report of Findings
On your next visit we request you bring your loved ones with you, as we will present a report of our findings to you, which includes the analysis of your chiropractic postural X-rays, your customized doctor’s recommendations, and financial options.
The Road to Health and Healing
Now that we have developed recommendations unique to your needs, we begin the road to healing and optimal health through neurologically based chiropractic corrective care.
